A 33 year old professional bodybuilder man presents with sudden right hemiparesia. He complained of malaise ,headache, night sweats, shortness of breath and cough for two weeks. After hospitalization regarding to the result of brain CT scan that showed left sided ischemic stroke, thorough evaluation began to find the cause of the stroke. Transthoracic and transesophageal echocardiography revealed infective endocarditis superimposed on a congenital bicuspid aortic valve. Stroke as a complication of infective endocarditis (IE) results from the embolization of endocardial vegetations with consecutive occlusion of an intracerebral artery. After the full course of treatment he discharged in good general condition, except for his hemiparesia with 4l5 muscle strength in right upper limb. He went to the stroke rehabilitation center where combination therapy program (conventional rehabilitation combined with virtual reality rehabilitation) was planned for him. This program included Recovery Tech VR game based rehabilitation to improve motor function in affected upper extremity.. Fortunately after eight weeks of VR therapy muscle strength scale showed significant improvement and he recovered enough to return to the gym but with lighter workouts.
